HIPAA Notice of Privacy Practices
THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN ACCESS THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

If you pay for a healthcare service entirely out-of-pocket, you may request that we not disclose information regarding that service to your health insurer for payment or healthcare operations purposes. We will honor such requests unless disclosure is otherwise required by law.
Receive an Accounting of Disclosures
You may request a list of certain disclosures of your health information made during the six years prior to your request. This accounting will not include disclosures related to treatment, payment, healthcare operations, or disclosures specifically authorized by you.
One accounting per 12-month period will be provided at no charge. Additional requests may be subject to a reasonable, cost-based fee.

You may request a paper copy of this Notice at any time, even if you previously agreed to receive it electronically.

Appoint a Representative
If you have designated a healthcare power of attorney, legal guardian, or other authorized representative, that individual may exercise your rights and make decisions regarding your health information to the extent permitted by law.

If you believe your privacy rights have been violated, you may file a complaint with In Motion Physical Therapy, PLLC or with the U.S. Department of Health and Human Services Office for Civil Rights.
